 IT IS THE STUDENT'S RESPONSIBILITY TO DISTRIBUTE THE FOLLOWING FORM TO THE SERVICE AGENCY SUPERVISOR  AND RETURN IT TO HIS/HER THEOLOGY TEACHER TO OBTAIN CREDIT FOR SERVICE PERFORMED.

After the school year begins, the Evaluation Form may be e-mailed, mailed, or hand-delivered to the appropriate theology teacher. Bishop Ready High School is not responsible for evaluation forms delivered to the school during summer break. Please wait until the first day of regular classes to bring in evaluation forms.
