To apply for EdChoice, email the following documents to Associate Administrator Jeri Rod at jrod@cdeducation.org.

1. EdChoice Request Form (one per child)

2. Proof of Address, such as a utility bill showing both the service and billing address (no cell phone bills). Only one per family is needed. Click here for acceptable proof of address documents.

3. A copy of your child’s birth certificate

4. If your student is a new student to Bishop Ready, provide a copy of your student’s latest report card.​

After submitting your paperwork, you also must complete the Income Verification through the OH/ID portal or by completing the form found here.
